REMINDER: This is NOT an alternative to the main Activity Check thread that will be posted in the Notice Board by a librarian in the very near future. Even if you fill out this form for a character, you must still submit the same character in the Activity Check thread, else he or she will be deemed inactive and removed from the directory.
