Tribute Leaving Thread

So I have a feeling this will become a necessary addition to summer Games. For the 58th I made a tribute leaving thread, where anyone going on vacations or trips during the summer with limited/no internet can post their dates so that I have one central location to check to see when people are gone, rather than having to sort through numerous PMs. I'd like to do the same for these Games.

If you play a tribute and you know you'll be going away for any amount of time greater than 2 or 3 days with limited to no internet, please fill out the following form and post here.
